Request for Social Assistance

Directorate for Health and Social Welfare

Purpose: Acknowledgment of the right to social assistance for families with poor material conditions.

Description: The applicant submits the request for family social assistance. Enclosed with the application the applicant provides the necessary documents for reviewing by the commission. The commission verifies the documents and factual situation of the family and the decision on the granting or refusal of the request is rendered.

  • Reports of 3 physician specialists. (Original)
