Correction, change in the Civil Status Register

Purpose: Correction/change in the Civil Status Register.

Description: The applicant files a request to the Civil Status Office. Upon applicant's request, the officer fills out the form and requires that all documents required for correction are completed. The application is examined and a decision for correction is issued.

  • Certificate of No Pending Cases (Original)
  • Evidence of criminal past (Original)
  • Other documents as necessary, if requested by the officer in the procedure. (NONE)
