Death Certificate

Purpose: Issuing death certificates

Description: Requests for death certificates are made at the Civil Status Office. Request may be submitted by a relative, a n institution or a person who was present when death occurred. The deceased must be registered in the electronic system of civil status, to enable issuing the death certificate.
